1. A valid photo ID(PASSPORT/US DRIVER'S LICENSE)

2. Insurance Card (If Applicable)

3. Vaccine records (If Any)

4. Green Card/immigration physical Exam Form I-693.

Please read the instructions below carefully before completing the form

Instruction for Completing I-693:

Do Not Sign/Date the form before seeing the doctor.

Use Black ink Only.

For Adults: Complete Parts 1 and 2.
For children under 15 years old: Parent must complete Parts 1,2, and 4
For patients who do not understand English: the Preparer must complete Parts 1, 2, 3, and 4.

*Please ensure that a barcode appears at the bottom of each page of the I-693 form, with your name at the top of each page.

Important Information Regarding Form I-693 Submission and File Retention:

Form I-693 must be submitted directly to USCIS by the applicant or their attorney. The clinic is not responsible for submitting this form on your behalf.
Sealed forms can only be picked up by the applicant in person.
For patients who do not respond to phone calls and do not return after their initial visit, we will retain their files for a maximum of 3 months. After this period, the files will be discarded.
